Spell Casting Request Form

Please fill out with the form with absolute truthful information - otherwise your spell will not work.

Please allow up to 48 hours for me to get back to you with details for your spell. This is not to cast a free spell, I am simply giving away as a holiday gift the psychic reading I normally charge to see if a spell will help you.

Instructions:

1. Provide you're real full name and correct email address so I can email your reading.

2. In the message box provide the following (all must be present to cast your spell)

(i) The FULL NAME of the person(s) you wish to cast the spell on.
(ii) Their birth date (month/day/year) if known.

(iii) YOUR birth date (month/day/year). Must be real otherwise the reading will not come out right.

(iv) Describe exactly (a) what the problem is and (b) what you desire the spell to do.

You must be 18 years old and older to have a spell cast for you - no exceptions.
